hi.
dilated cardiomyopathy vs restrictive cardiomyopathy vs constrictive pericarditis.how to clinically diff between them?sometimes i just get stuck

Dilated: can't pump
hx alcohol(#2 mcc, #1 is idiopathic), or infection (coxsackie)
decreased CO,SV,
si/sx LVF & RVF = pulm edema, jvd, HSM, S3
dx: echo dilated LV, decreased wall motion

Restrictive CMP: muscle cant move, heart cant fill in diastole
hx infiltrative dz: amyloid, sarcoid, fibroelastosis etc
echo: thick ventricular wall (ddx constr.pericard=pericard thickening)
CT: normal pericardial thickness

Constrictive pericarditis: cant fill in diastole
hx pericarditis, radiation, heart surgery, TB(immigrants), viral infxn
si/sx: distant heart sounds, "pericardial knock"
CT/MRI: thick pericardium, pericardial calcifications
no ventricular wall thickening.


sometimes may require biopsy to differentiate constr pericarditis - restrictive CMP.
