Hi
I am sharing my list of neurology programs. It contains information about most of the programs (except california). The information is related to what i though was necessary for my decision to apply. The information was current till October 2007. Still i take no responsibility as to the accuracy of information and am not responsible for errors.
The list was not made in excel sheet though i am giving it out as excel sheet with the hope that most of you have MS Excel installed. If you need it in any other format i might be able to help you. Though excel sheet is probably better to handle. It was made in a database program specially designed for me by my brother. The program can generate lists by information filled in as forms. These forms can be created, modified, corrected easily. The list generation was with the most current data. The list could be manipulated in different ways and could be searched in different ways using multiple filters. The database could even be put online and easily accessible from internet. I think although it took him only 4 days, it is an idea and effort which should not go waste. I am trying to convince him to put this software online where people can make an account and use it online. It helps to keep track of the whole application process. Drop me a line if you think this idea is worth working on.

This list contains information obtained by program websites and emailing to programs. It also contains my personal interpretations. Here is a brief description of the columns:
Decision: my decision to apply or not to apply
Current IMGs: as estimated by their websites resident pages
Matched; If any of my friend has matched in the hospital , not specifically in neurology.
Prelim available: Does the medicine program in the hospital has prelim positions
Program type: obviously either integrated or non-integrated or some times not actually integrated but have a close coordination with the medicine program and get you an interview.
Seats: obvious (some programs have increased their seats as i got to know in my interviews, i can tell for sure about university of nebraska and university of oklahoma and university of kansas)
US clinical experience: obvious
US News Rank: obvious
Visa: as given on their website or as given in their email replies . this issue does however change for some program every year, atleast reconfirm with your selected program if it is that important to you, personally i applied to both J1 and H1b programs but ranked H1b programs higher (reconfirm the visa sponsored during interview).
Combined: If any combined specialty residency offered at the hospital. Of note however is that usually combined residency is not being offered by department of neurology itself rather by the other specailty involved and you thus submit application to that department for combined program. This list however is incomplete as i got disinterested in it.
Contact info: many of the email addresses here are very current and you will not find anywhere else. As sometimes you get reply from a different more active email address and i changed the list to that. and sometimes the frieda email was bouncing and so i searched the website for it, sometimes even in the faculty database. but still there are some emails missing.
Email: If they replied to my email or not. This list may not be the perfect because i remember in the end most of the replied and i forgot to update the last replies.
Fellowships: In neurology there are 9 accredited and non-accredited fellowships. I have just wrtiten the number of fellowships as writing the actuall names was cumbersome. It was to have an idea of stregnth of faculty as more the fellowships offered, better the faculty and better the program.
Moonlighting: obvious
Program based: There are hardly 5 programs i think which are not university progam and even those are good ones like cleveland clinic and it should not be of any matter.
Ranking: this is my opinion after reading reviews at scutwork.
Requirements: any specific requirements.
State: obvious

Yes, if you are applying to advanced program you need also apply for prelim positions. It can be in the same institution of in different one.
