i have a question please:
while taking social history n pt says he s been smoking for x yrs, is it appropriate to do the counselling at that juncture or wait till closure?

replies would be appreciated,thanks

If you don't forget-do ot at closure
but most of the people forget that-so better to do it at the same of SH

do it in social history and score a point....dono test urself and dono wait for closure

I actually did both.........

On some cases where I thought I would forget or would not have time at the end..........like a CNS problem, I counseled them as soon as they told me about their smoking habbits.

On others I waited till the closure......Definitely have a plan as to when you are going to do what but make sure that your plans are not set in stone. Be able to adapt to the flow of the history and the type of patient and adjust accordingly.

If you counsel right then, you may sound judgemental. If you dont, you risk forgetting it.

Whenever i found anything on the history that I would like to counsel about, I would put a big circle around it in my notes. This way I never forgot to counsel about anything that i had asked about ( ofc i might have forgot to ask a few questions). But, I always did the counselling at the end or while i was answering the 'challenging question'

I counseled about bad things at the end, and praised right away (a patient following a good diet, for example). I think counseling right after the patient tells you about a bad habit sounds harsh and makes the patient want to withhold information about other bad habits. At the end, he's generally more receptive to advices.
