HCTZ unless PT is black african-american (the most common used abbreviation of hydrochlorothiazide is HCTZ not HTZ)

Hi Pulse pressure (systolic - diastolic) = Stroke volume /Compliance In old person compliance is reduce -----------> increase in PP i.e systolic is increased and diastolic reduced.It is called as "Isolated systolic HTN". Now if you look at above equation ,the best strategy should be to reduce "stroke volume". Thiazide diuretics can reduce the SV by depleting volume (in addition to vasodilater action) but anti-HTN CCB which are usually Dihydropyridine do not reduce the SV but are only peripheral vasodilators. Any pure peripheral vasodilator will ultimately lead to "fluid and Na retention" necessitating the need for some additional diuretic. So it should be "Thiazide diuretic"

Agree. HCTZ is the first choice unless compelling indications present from other drugs for specific conditions.
