Please Advice J1 research to J1 clinical

Hi everybody,

This is my story, I've been doing research for the last 2 years under a J-1 research visa, sponsored by my Hospital, not by the ECFMG, I am not subject to the 2 years home requirement.

After my third attempt in the match, I finally got my PGY-1 preliminary position for general surgery.

Here comes my problem: I was happily applying for the ECFMG J1 clinical visa, until I've got an email from the case manager asking me for 4 letters explaining and supporting the change of status and also explaining me some more things.

I will quote some parts of the email:

"Please include detail on your plans beyond the PGY-1 preliminary year; it is important that you document your understanding that, if approved for the requested change in category, you would not be permitted to repeat your PGY-1 surgical year should a categorical position become available (note that your letter should confirm your understanding that, if approved, you would be required to progress in PGY level each year and complete a surgical residency in no more than five years)"

"It is important that your proposed program director document his/her understanding that, if approved for the requested change in category, you would not be permitted to repeat your PGY-1 surgical year should a categorical position become available (note that this letter should also serve to confirm his/her understanding that, if approved, you would be required to progress in PGY level each year and complete a surgical residency in no more than five years)"

1) Does anyone knows if they are giving me 5 years because I already spent 2 years on a J1 research visa? instead of the regular 7 years to finish the residency for a normal J1.

2) Can I go back to my original country and apply for a J-1 Clinical from there? (obviously after ending my actual J1 research)

3) How lond goes it takes to do a "regular" J-1 Clinical visa through the ECFMG from outside the US?

Any other suggestion?
