Hi,

I am taking 2 full months to prepare for my CS Exam. I am full time student so I hope 2m is enough time to nail this test. Anybody in the same boat? Join me! This will be my

I will make use of Kaplan CS Handout and Red Book and UW. I also like the tips from this website: http://csprotocol.blogspot.com/ Take a look if you can.

Getting Ready for my 1st Week nod

Tomorrow Monday I will start with a fresh reading in the morning from Kaplan 5 Days Handout and Red Book & First Aid for the Step 2 CS 2007. (Introductory and First Chapters from those books). I hope to be done with the initial theory in 3 days.

Lainie, thanks for the advice but I took kaplan course 5d sometime ago and I will just review what I already studied; FA to practice the cases definitely and UW for cases not presented in FA.

Well, back to Business here

Somebody told me that patients (SP) make you fail or pass and that is almost true, because if we see:

(CIS) Communication and Interpersonal Skills ----- Evaluated by the patient (SP)

(SEP) Spoken English Proficiency ----- Evaluated by the patient (SP)

(ICE) Integrated Clinical Encounter, which contains: History (20%) Physical (20%) Patient Note (60%) ----- History & Physical are evaluated by the patient SP & Patient Note is evaluated by a physician.

It doesn't look that bad huh?, we don't have to be perfect, we just need to passsss...



Ok, Let's get this started ......

Clinical Skills Exam

Each Case: 25 min

Outside the Room: (X seconds)

- Patient Doorway Information (Read it and make some notes in your blue sheet)

Inside the room: (15min - X seconds)

- Introduce yourself

- Bedside manners (CIS)

- Take relevant medical history

- Wash your hands

- Focused physical exam

- Closing or summary

Outside the room: (10min)

- Write Patien Note (10min)

Time Management:



15min in SP Room


"Examinees, this encounter may now begin"

- Doorway Information: 30sec

- Introduction & History: 7min

- Physical Exam: 5min


"There are 5 min remaining" (You better be in the middle or ending your PE, if not, start already!)

- Summary and Closing 2min


"This encounter is now over"


10 min for PN


Leave the room and start your PN: 10min for Patient Note


"Pencils down" (Stop! they really mean it)


That's it!

Grechita, let me know when you are in chicago, we could do live practice nod I just hope you are staying close to the test center.

Sahashra, I don't have the link for the book if that's what you mean, a soft copy, sorry about that sad and I got mine when I took the 5d course.

OMG somedays I feel so sick that I wish I could shut myself down for 4 more months but I can't afford it, so I better get this going...

What tasks you need to complete (sometimes they don't ask for PE)

Write your Diff Diagnoses

Self Introduction



"Mr/Mrs ..... ?"
"Hi, I am Dr. House" Handshake if SP is not in distress.
"I am the physician on duty here today"
Drape the pt. "let me make you more comfortable"
"I hope you don't mind If I sit, while I ask you some questions"


Straight to Business... Collecting SP's information.
"HISTORY OF PRESENT ILLNESS"


Start with an Open - Ended Question : "How can I help you today?"
Pay attention to the patient, make eye contact, remember "clipboard is not more important than the patient"
Focus on the Chief Complaint (CC)

Mnemonic for HPI:
S : Site / Symptom
I : Intensity / quantity
Q : Quality of the symptom
O : Onset
R : Radiation
A : Aggravating factors
A : Alleviating factors
A : Associated symptoms


PAST MEDICAL HISTORY:
Mnemonic for PMH:

P : Previous episodes of CC
A : Allergies
M : Medications
HITS : Hospitalizations, Illnesses, Trauma, Surgery
RUGS : Review of systems (Urinary, GI, Sleep.
F : Family History
O : Ob/Gyn
S : Sexual history
S : Social history, habits


Don't forget Transition Sentences before: Past Medical History, Family History, Ob/Gyn, Sexual and Social History; very important.


When done collecting all this information, Transition sentence before doing the Physical Exam: "Thanks for answering all these questions. Now I’ll need to do the physical exam, So I’ll just wash my hands first."


Now, straight to the cases ...
