case info..

age=50 yrs, male
non-hypertensive,non-diabetic,
smoker....10 pack years of smoking

presenting complaint......bleeding from gums, and epistaxis for last 15 days

complete blood picture 

hb= 13.6 gm/dl
esr=10mm after 1st hour
tlc= 4600/ul
neutrophils= 30%
lymphocytes=67%
eosinophils=2%
basophils=1%

retic count =1.5%


platelet count 39,000/ul

clotting profile
prothrombin time....14 sec (control 14 seconds)
APTT = 42 SEC ( control 42 seconds)
bleeding time and clotting time also in normal ranges

peripheral film

normocytic,normochromic...neutropenia and thrombocytopenia


bone-marrow-analysis (biopsy )

noraml

on examination....massive hepatomegaly ( 25cm in vertical dimensions) ,spleen palpable in left hypochondrium only..( mild splenomegaly )


whats the diagnosis?....are we missing something in labs?

bleeding is explained by thrombocytopenia,massive hepatosplenomegaly suggest parasitic infection etc affecting mega lineage, ur not fully clear what u want ask
