If BB is < 10mg there is a good chance to diagnose biliary artesia with HIDA scan after 5 days of phenobarbital. The test should reveal a normal uptake by hepatocytes but failure of isotope excretion in intestin after 24 hours. Liver biopsy to confirm the diagnostic. US to exclude a stone/cyst Blueprint page 202

 Phenobarbital Role: phenobarbital stimulates the activity of glucuroyl transferase and the synthesis of Y and Z carrier proteins and therefore increases uptake and conjugation of bilirubin a 3-7 days course of phenobarbital therapy increases the sensitivity of HIDA scan in differentiating between biliary atresia and other causes of neonatal cholestasis

Does a person have to hold still for a long time during a HIDA scan? Is a HIDA scan as good as an MRI? Why did my gastroenterologist not mention a HIDA scan to determine if there is a gallstone blocking my bile duct? He only mentioned an MRI. LC

well hospitals have different management techniques. these answers r based on usmle preference, that doesnt necessarily mean actual hospitals do these things in reality. another thing i am sure u r not 8 mo old, i mean this is for pediatric population
