A 34 year old woman is evaluated for diffuse pelvic pain. Her husband was treated one month previously for a sexually-transmitted infection, but she was not treated at that time. Now she is febrile and has a tender mass around her left adnexa. What is the mechanism by which the most likely causative agent may cause fallopian tube scarring?

a) Cytotoxic T lymphocyte -mediated cytolysis

b) Immune complex -mediated cytolysis

c) Neutrophilic infiltration

d) Stimulation of IL-8

e) Type II hypersensitivity

the reason i chose A is "scarring" is a chronic process........and in chronic process "lymphocytes" are involved......no?confused

Correct answer: A

EXPLANATION

This woman has developed Pelvic Inflamatory Disease, one of the sequelae of untreated STD. Ascending infections involving either gonococci or Chlamydia may enter the uterine tubes, producing salpingitis, and eventually infecting the fimbriated ends and the ovary. In the US,Chlamydia trachomatis is the most common bacterial sexually transmitted disease.Chlamydia are intracellular pathogens, and thus elicit a TH1 lymphocyte response and stimulation of CD8+ lymphocyte effector cells. The damage and scarring of the fallopian tubes, therefore, is at least in part due to the development of a type IV hypersensitivity reaction.

Neutrophilic infiltration (Choice C) is a common mecahnism of abscess and pus formation in cases in which extracellular pathogens invade the tissues. Since Chlamydia are intracellular organisms, they would not stimulate the influx of neutrophils.
