I liked this Mneumonic so adding this to my Journal -- not to miss in later part of my preparation
The phrayngeal Pouches i created a formula for them ...
1A, 2P, 3 TIP, 4 SPUB
It might seem even more difficult but let me explain ...
1A .. 1 for the 1st letter of Alphabet which is A stands for Auditory ... So, 1st Pouch gives the epithelial lining of Auditory tube and middle ear cavity
2P ... 2 for the 2nd letter of the Alphabet which is the P ... for Palatine ... So, 2nd pouch gives the epithelial lining of cryPts of Palatine tonsil
3 TIP ... 3 then 3 letters TIP start with T of Three ... these letters are the abbreviations given in kaplan for those derivatives and stands for Thymus and Inferior Parathyroid gland
4 SPUB ... 4 then 4 letters SPUB ... Also the abbreviations given in the notes for Superior Parathyroid gland and Ultimobranchial Body
The Arches ...
The Nerves ...
March 1975 i.e. 1975 = X, IX, VII, V and March to remember it is V3 ....
Arrange them .... Arch 1 gives V3, 2 gives VII, 3 gives IX and both 4 & 6 will have X
The Arteries ...
1st - part of MAXillary a. -------------- 1st arch is MAXimal
2nd- Stapedial a. & Hyoid a. ------- Second for Stapedial
3rd- Common Carotids and parts of internal Carotids ---- C is the 3rd letter of Alphabet
4th- on the lt. --> AORtic Arch and on the Rt.--> Rt. Subclavian ----
4th arch (4 limbs) = Systmeic & Also remember that AOR is from FOUR
6th- Pulmonary arteries and Ductus arteriosus ---- 6th arch= Pulmonary and Pulmonary - Systemic Shunt ( Ductus )
Muscles (Mesoderm) & Skeletal (Neural Crest) Derivatives ...
1st arch ...
Muscles => V3 innervates M.D. My T.V. i.e.
Muscles of mastication ( masseter, temporalis, pterygoid)
Digastric ( Anterior Belly)
Mylohyoid
tensor Tympani
tensor Veli palatini
Skeletal => I'M getting Skeletal like Silvester
Skeletal remind u of skeletal derivatives then
I for the 1 i.e. 1st arch and for Incus
M for Malleus/Maxilla/Mandible
S of Silvester for Sphenomandibular ligament
2nd Arch ....
To remember the Muscles innervated by the CN VII ...
Imagine someone making the Facial Expression to say " PSS ... "
Facial Expression muscles
Posterior Belly of digastric
Stapedius ----> and it reminds u of the other skeletal derivative Stapes
Stylohyoid -----> and it reminds u of the the Hyoid ... but not all the Hyoid, we still in the 2nd level so choose the lesser that is above i.e. the Lesser Horn of the upper body of the Hyoid.( and when u get to the 3rd arch do the opposite )

Hai All
Visiting my Journal infrequently
But now on, I will be visiting my site on daily basis .I will try to update my Journal daily.
I am still doing Pharmacology. I have completed 36 chapters from Katzung Review. Read Twelve Chapters of Unit V: DRUGS THAT ACT IN THE CENTRAL NERVOUS SYSTEM.
These Chapters include:
1.Introduction of CNS Pharmacology.
2.Sedative-Hypnotic Drugs.
3, Alcohol.
4. Antiseizure Drugs.
5. General Anesthesia.
6. Local Anesthetics.
7. Skeletal Muscle Relaxants.
8. Drugs Used in Parkinsonism & Other Movement Disordes.
9. AntiPsychotic Drugs & Lithium.
10. Antidepressants.
11. Opoid Analgesics & Antagonist.
12. Drugs of Abuse.
Still to do following two Units from Katzung Review:
Unit VII: Endocrine Drugs.
Unit VIII: Chemotherapeutic Drugs.
Next Target is to complete Microbiology & Immunology + Chemotherapeutic Drugs in 2 week time.

1. Kaplan Microbiology & Immunology Lecture note along with Video Lecture.
2. Rapid Review Microbiology & Immunology.
3. Microbiology & immunology Flash Cards.
4. Chemotherapeutics from USMLE Road Map Pharmacology.

Well calculating in real time frame, It looks to me 20 days. Let’s see- today is July 9th, 2008. Studying from July 10th , 2008;
- 2 days for Kaplan Video Lectures.
- 10 days for Rapid Review Microbiology & Immunology + Chemotherapeutics from USMLE Road Map.
- 3 days for Chemotherapeutics from Katzung Review.
I will try my level best to keep myself tight upto Schedule.

These r some clues might help u ...
Fever + neck stiffness = Meningitis
Fever + Altered mental status = Encephalitis => mcc HSV
Fever + Focal neurological deficits & CT brain showing contrast nhancing lesion = Check HIV status => if +ve then either Toxoplasmosis or Lymphoma, if -ve => biopsy to differentiate infec. from cancer
Polymyositis Vs Dermatomyositis = Both Myositis i.e. Proximal ms weakness i.e. can't comb hair, can't get up from chair + elevated ms enzymes CK & Aldolase ... In Dermatomyositis as the name implies has additional skin rash ... don't bother urself what type it is .. however it is bretty characteristic ...
Fibromyalgia = pain and weknes of proximal ms too but in old pt > 60y/o and ass. with a very high ESR
Polymyalgia Rheumatica = pt. c/o Insomnia, easy fatigability & widespread musculoskeletal pain & stiffness ... Key -> up to 18 ( at least 11) aymmetrical bilat. tender points that occur in the same location in all pts .... the Qs won't give u 11 ... just will tell that pt is c/o multiple pain and will mention Insomnia and [i]Labs are NORMAL ...
MS = mutliple neurological deficits separated by space ( anatomical) and time i.e. 1 now, 1 a couple of months back and can't be explained by single lesion ... Optic never affection is very common ..
Myasthenia Gravis => Ms weakness at the end of the end while Eaton Lambert => Increase Ms strength at the end of the day ...
Remeber that both MS & MG are demylenating dses one central affecting Oligodendrocytes (MS) and the other is peripheral affecting Schwann Cells ( MG)
Guillain Barre => Descending Flaccid Paralysis while Botulism => Ascending Flaccid Paralysis
To remember GuiLLain Barre Goes from LL to Brain

u know what.....one Q in boards came from the polymyo and dermatomyo thing.....add the immunopatho of the disease in that as most ppl forget to cram that thing and that is where the boards test us on!

Yeah u r right after completing subject i feel quite confident for that subject but first read takes time and then lots of distractors in terms of STEP I Prep slows down my first read as you said in ur preparation 2-3 fast reads then a slow one.As I asked to ALLAH:

Hai Everyone there
Conceptualised Kaplan Immunology Videos. Marvelous. This Don is from USA. He is Don of Immunology.
I will do Immunology from Kaplan Notes today.
However confused between Rapid Review and Kaplan Lecture Notes. I have decided to read both books as last read will be from Kaplan Medessentias and First Aid.

Not upto mark . I bought laptop today for this USMLE and Kaplan and Mr Goljan audios, Slides and Goljan notes.This LapTop will accompany me till match after which i will buy costliest LapTop in Town ---Intel core extreme inside Dell XPS M1730 or Lenovo blah blah blah . Last week friends wedding, Tommorow cousins wedding --- Lot ot distractors. Hopeful to start again on 1st of August from Immunology again.
After starting kaplan I feel that I have to restart again my Preparation as I left Kaplan out from my preparation till now but i really feel that Kaplan material is best for USMLE and leaving it will be a foolish act as SCORES do matter.
Reading a chapter a day from Kaplan Immunology among all these distractions though i will sum up all the chapters of immunology in a day or two again
Material to be used for Microbiology & Immunology -- Kaplan + Rapid Review.
