A 35 y.o. woman has a several year history of relapsing and remitting neurologic symptoms including paresthesias, visual symptoms and mild motor symptoms. Which type of hypersensitivity reaction is associated to patient disease?

a) Type I

b) Type II cytotoxic

c) Type II noncytotoxic

d) Type III

e) Type IV

EXPLANATION

Multiple Sclerosis is characterized microscopically by plaques with a central loss of oligodendrocytes and myelin but with relative preservation of axons. The plaques are surrounded by peripheral rim of lymphocytes and increased astrocytes.

The disease appears to be initiated by CD4+ TH1 lymphocytes that react against self myelin antigens and secrete cytokines such interferon gamma to activate macrophages. The activated macrophages are responsible for the demyelination. The disease is therefore classified as an example of a type IV hypersensitivity disease.

Bro i answered on the words of Goljan. RR pg 580 Pathogenesis of MS and all the other articles also state that there is a presence of MBP antigen and CD 8 and CD 4 Lymphocytes react to it. So i thought this is an Antibody mediated Cell Cytotoxicity (Type 2) rather than Antibody independent T-cell activation (type 4)

From RR pg46:

Type IV hypersensitivity

Antibody -independent T cell-mediated reactions (cellular immunity)

1.-Delayed reaction hypersensitivity

CD4 cells interact with macrophages (APCs with MHC class II antigens),resulting in cytokine injury to tissue.

2.- Cell mediated cytotoxicity

CD8 T cells interact with altered MHC class I antigens on neoplastic virus-infected, or donor graft cells, causing cell lysis.
