Took the exam yesterday and today. Here is how it went: first day: thought I did horribly on the multiple choice - there were about 10 questions that I knew the answer to and still put the wrong answer second day: thought I did very well on the multiple choice. Here were my CCS cases and how they went, which I thought was rough: 1st - do not know what the diagnosis was, but the patient stayed the same and the case ended at 10 minutes turner's syndrome - ordered all the basic labs and prescribed estrogen for the patient, the case kept going, once I ordered OB/GYN consult saw that I forgot the growth hormone so ordered that and prescribed growth hormone replacement, then the case ended diabetes - why did i screw up this case? the guy comes to clinic with a glucose in the 400s and positive symptoms so I send him to the ER. I make him NPO and give him normal saline, yet the labs show no anion gap, etc. I order multiple BMPs until I realize that I need to give the patient IV insulin which the patient does so much better at 30 hours simulated time - about 15 minutes my time pancreatitis - i did everything but order the GI consult, but the case kept going until the patient did better - this was done at 15 minutes cervical intraepithelial neoplasia type III - done with this case as soon as I ordered the colposcopy and cervical biopsy - about 5 minutes hemolytic uremic syndrome - sent the patient to the ER, ordered prednisone, plasmapheresis, but then the patient did well after two dialysis episodes - case ended at 7 minutes ventriculoseptal defect - case was done at 4 minutes as soon as I ordered cardiac cath and pediatric cardiology consult - did not medically manage patient except thought the kid had RSV and gave ribavirin foreign body aspiration - just got a portable but ordered a bronchoscopy as soon as the physical exam suggested it. ended this case with the patient in the ER - ordered albuterol, chest physiotherapy, and check pulse ox some case where the guy just received opiates for a dental procedure and had cirrhosis - as soon as the guy got worse at about 10 minutes my time the case was done, and I did everthing including continuous mucomyst, which i ended up ordering late I hope it goes well, this is my second time taking the test
