If I do an elective in a certain field and obtain a good US LOR then decide suddenly to go to a different specialty for residency , will my LOR and USCE still count -given that I can not go for another elective or observership after graduation- . Do I have to stick to the first choice just because I did a USCE in it? Input plz

Hi, i don't know the answer of your question but I think USCE is USCE. Of course it would be better if it is in the same field you are pursuing but still it should count. For instance when i was medical student (a long time ago...) I participated in a clerkship in IM and ID here in US so I don't understand what would be the problem in using an ID rotation LOR. Let's wait for more input.

Thanks Ivonne the most important part is that will the LOR be taken seriously if it's from another specialty and let's say it says really good things about you waiting for more input

How far are the specialties though? let say surgery LOR and IM residency. It is more difficult to buy it... It sounds better IM/ID/cardiology/more because even the IM resident rotations includes specialties right? and ultimately after IM is the fellowships.

A reco letter is meant to appraise the PD of your ability to be a good physician in the US. It's not meant to convey that you're good at the field already, that's what residency is for. While it's better to have a letter from the same field, a letter from any other field will certainly have some value, definitely not worthless. My $0.02.

If you are applying for a very competitive field, LOR from physicians in that field would be very important. But if you go with IM, and you have USCE in family practice or visa versa, you will be fine as long as you are spoken highly of. Anyway, whatever LOR in the USA you can get, it would be better than a LOR from your own country. My two cents. GL.

If you go for psychiatry A friend told me that they ask him in the interview, why don't you have any psch usce?

can any one clear my doubt in this matter. If i wish to get into prelim surgery will doing an obsevership in psychiatry will help me becos after trying so hard for the past one month i ended up getting one in psychiatry.

I also have same doubt.I wanted to do IM but I might get observership in surgery.But thats the only one I can get.Also another doubt is about externship.If i get externship in a non teaching hospital and observership in a teaching hospital which one is better?

Naturally, it would be better to have USCE in the same specialty you're applying for in the match but this doesn't mean that if you get an opportunity to gain - regardless of the field - that you turn it down just because it isn't the same field. You'll get US LORs' and exposure to the US medical system. In competitive specialties like surgery and dermatology, it would be better - some even say crucial - to obtain relevant USCE. Nevertheless, if offered any elective or externship in the US... go for it! I'm not expert, just using common sense here.

My experience at the electives I did recently, that the doctors there are not assessing your knowledge in a particular field, but rather your adaptability and understanding of the system, and also importantly your approach to medicine and the ability to learn. If a LOR is there from a good enough doctor or place, approving all this, then the field should not matter... but at the same time these basic qualities can vary from fields relating to surgery and that of medicine.
