39. A 52-year-old man with a history of diabetes
and hypertension presents to a new primary
care provider. He currently uses insulin to control
his diabetes and hydrochlorothiazide
(HCTZ) to control his hypertension. On physical
examination, he is an overweight man
with no concerning findings, except for a
blood pressure of 171/93 mm Hg. His creatine
is 1.7 mg/dL with a urinalysis that shows proteinuria.
His physician wants to add another
antihypertensive in addition to his HCTZ.

40. A 63-year-old woman with a history of osteoporosis
presents for a second opinion regarding
hypertension management because she is worried
about starting any medication that might
worsen her osteoporosis. Her only medication
is calcium supplements. She smokes 1 pack
per day for 15 years. Her physical examination
is normal, except for a blood pressure of
165/90 mm Hg. Laboratory work shows total
serum calcium of 7.0 mg/dL and mild hypercalciuria.
She wonders if an antihypertensive
exists that would not cause her calcium to decrease
further and that might even increase it.
