40 year old man hallucination for 24 hours, binge drinking 3 days ago. Type 2 DM and hypertension. take unknown medicine. T: 100 pulse: 110 BP: 165/100. Liver 16cm and tender. Lab:
Hb 11.2
MCV 103
WBC:5000
platelet: 50000
pt 15 sec
blilrubin 1.3
ast 120
explanation for these finding?
1. drug interaction
2. hypersplenism
3. myelophthisic marrow
4. peripheral destruction of cell
5. suppression of bone marrow production

I think megaloblastic anemia with reduced production of all cell lines.

I think it is the effect of alcohol in the bone marrow then 5555555555555 Suppression of the bone marrow.

The answer is-5
and the only reason for that is ALCOHOL DIRECTLY SUPPRESES BONE MARROW and subsequently causes pancytopenia.

i thought it was 2 , the WBC is low? whats the normal range again plz?

wbc 4.5-11

megalob = anemia from alcohol & vitamins
