35yo female with epigatric discomfort for 2 month. No nausea vomiting weight loss. No relation to meals. FOBT negative . Blood counts, serum chemistires normal. History of gastric ulcer in mother. What is the next step in management?

Ba swallow

Endoscopy

Trial with PPI

H pylori breath test

Trial with antibiotics

Aint so easy friends, the answer is H pylori breath test!!

In other words, what are the indications of H pylori testing???

Kaplan said we can give trial of PPI in cases like above mentioned case.
I asked a GP practicing in US about tests they do before giving PPI/ eradication regime.
They did blood test/breath test to get the H.pylori status.
If positive, give eradication regime.
If negative, trial with PPI.

More inputs please.

from UW: if pt have no risk, test H pylori then try PPI

(BUT if it's only a nonulcer dyspepsia, H. pylori eradication is not very useful. So, not clear cut.)
