In case u have exhausted your slot extension once, u will need to sit and contemplate on whether u r ready to take it or not... Best way wud be a NBME assessment.

In case u wish to write, go on with full confidence.

In case u wish to postpone, forget abt sitting for the exam on 28th April. NO hell will break lose if u dont and cant write exam on April 28th. However u will need to reapply and get a new permit for 845 $ in case u r writign outside USA. And one more thing is that one cant reapply straight away after their extended step 1 slot expires, There is a 2 week wait period after which ur application will be accepted.
