Counseling transitional phrase

I have 2 Qs:

1. what is the transitional phrase you use before you start counseling

2. at which point of the PE you start the counseling part

after you are done with the examination in a very professional yet cordial way....you can say something like... Alright Mr/Mrs...... I have completed the physical examination and I think I now have all the information I need, Now I'd like to sit down and discuss with you the possible diagnosis and how I'd like to go about your case...and then begin with DDs along with conrresponding tests to confirm/rule out and then counselling against alcohol/smoking/lifestyle changes...

Too lengthy... You do not need to say that now you want to sit down and talk... You have to be concise but all-inclusive...

I suggest the following:

Alright, Mrs X/Mr Y,
the symptoms you described to me (repeat what you were told be SP) may be due to several reasons. My examination of you suggests that you may have (DD, nit more then 3 options), and now we need to run some tests to either prove or rule out these conditions. I ask you to consent for (usually list all invasive tests), and when the results of these tests are available to me, I will be able to tell you about your diagnosis, prognosis and treatment. Meantime, I would like to remind you to (counsel here). Do you have any questions to me? No? Thank you for coming, I will see you again soon...
