I'm newcomer to USMLE ... and I'm trying to apply to USMLE step 2 CK. I've never applied for any other USMLE step before.
I read in the ECFMG information bulletin that I have to submit forms 344, 345 and 186 and mail them to ECFMG in Philadelphia.
After downloading forms 344 and 345 from the ECFMG site I was happy until I found out that I cannot find form 186! Where is form 186 (Certification of Identification Form) and how can I download it ??? I cannot find the link to download this document from the ECFMG site. I'm in a complete mess...can anybody help me to find how to download this document ??

You have to apply first on-line then they will give you that form. Good luck.

Thank you Markus for replying. Do you mean I have to pay first for the exam online and then the form will be given ?

Probably, i did that application long time ago. If you are not sure give them a call and find out. I think you have to fill out your info of you and your medical school then the fee comes up...then they will tell you that your application is not complete until you send them the remaining documents.
But they are not going to verify any of your documents if you dont pay them first...

yes
first you have to complete the online application. at the end you will be given an option to download the form 186 that you have to complete and send it to them

You have to complete the application and pay first, then you'll have an option to download the form.
