Are you asking about Left sided CHF or Right sided CHF?

i think the answer would be different in both cases.

I don't think rales would be very specific, right?

I'll go with JVD.

The question is "most sensitive"?!

I am considering left sided CHF as much more common than right.

PE findings are little specific to detect compensated CHF.Based on article byRobert Badget(JAMA1997):

very helpfull findings:JVD and abnormal aplical impulse(both for systolic dysfuncion)

Hi, dr19! I agree with you, compensated CHF may have little findings. I was thinking about decompensated chronic heart failure, with a lots of symptoms, not necessarily a lot of signs.

Still... I can tell you, for the right sided CHF (with or without left sided CHF), we find much more patients with lower extremities edemas than JVDs. JVD happens before the edema in an acute event, such as cardiac tamponade or acute cor pulmonale. Otherwise, the edema and hepatomegaly happen before the JVD.

So... let's wait for khiladi's answer (I hope he/she has it!).

Just another little detail: JVD not necessarily means systolic disfunction, right? I mean, in cardiac tamponade, for example.

what abt S3, I believe S3 gallop is the most consistent Physical finding......

Maybe the most consistent, but it's not as common as rales and edema. The problem is everywhere I read there's a list of symptoms, I didn't find the information of THE most sensitive, THE most prevalent, THE most specific... So I am giving my opinion according to what I experienced with the tons of chagasic patients I had. Not exactly evidence-scientific-100% sure type of answer. smiling face
