A 35-year-old white male, who is known to be positive for human immunodeficiency virus, presents for infectious diseases follow-up, complaining of chest tightness and mouth soreness. He reports that he was recently in Ohio with his bird-watching club. A physical exam reveals ulcers on the tongue, and auscultation indicates possible lower lobe consolidation with egophony. Radioimmunoassay testing confirms the diagnosis. What is the morphology of the yeast form of this fungus? A. Alternating arthrospores and empty cells B. ‘Captain’s wheel' configuration, with small buds around a thick-walled central yeast cell C. Oval buds within macrophages D. Spherule with endospores E. Tuberculate macroconidia

The organism is histoplasma capsulatum. It exist both as mold in the soil and yeast in the tissue. The answer is letter C oval buds within macrophages it is the yeast form of the organism found in tissue. H. capsulatum has 2 types of asexual spores the tuberculate macroconidia and the microconidia. Alternating arthrospores and empty cells is the spore form of coccidioides immitis. Captain wheel configuration with small buds is found in partacoccidioides, spherule with endospores is found in coccidiodes immitis found in the lungs it think it is the yeast form of coccidiodes immitis. Goodlluck
