I just recently took the test in Chicago.... I was born and raised in the US so communication was not an issue for me. The cases were vague but not difficult, before going into the cases I wrote any pertinent vitals along with writing out HPI, ROS, Meds, Allergies, PMH/PSH, Social and Family hx, I did this to be certain I would not skip anything. Always walked in, shook hands and introduced myself. Always washed hands and cleaned the stethoscope. During the HPI I always asked the routine stuff like duration, location, intensity, alleviating/worsening, association with other symptoms etc. After gathering all the hx, I would put my paper down and summarize to the patient what the told me and then tell them what I thought they had and how we would try to find the answers. Here are some things I know I did not do:

1) I don't know why I goofed, but anytime I felt a patients abdomen or listened to the abdomen, I did so through the gown (ouch), even though I listened to the lungs and heart directly!!!
2) I did not go into detail with the physical exams, I mean now that I think of it, there were certain physical exam findings I could have tried to illicit.

I am a nervous wreck about this, I don't know why?!?!? I have had plenty of US clinical experience, I smiled and made appropriate eye contact, etc!!! Any advice would be appreciated

I think examination is a technical thing and SP can't judge if you missout minor details until and unless you didn't do something which was really required like CVA tenderness in kidney case. About examining through the gown, its really a tricky one. I think arlete can comment better on it, but didn't the SP object? Otherwise, I really thing that you did great.
