A 35-year-old man comes to your office with a complaint of recent development of headaches that are generalized in nature. He is accompanied by his wife who recently returned from a trip. She tells you that her husband has been somewhat confused at times and clumsy. He does not confirm this, but he does report that he has been short of breath at times. On examination, he is mildly tachycardia and there is a reddish appearance to his mucous membranes, which is subtle. The only recent new medical problem identified is that the patient was in a motor vehicle accident in which he was struck in the rear end, and it left him with a sore neck. Which of the following is the most likely diagnosis?



A. Acquired spinal stenosis (cervical)
B. Normal pressure hydrocephalus
C. Carbon monoxide poisoning
D. Cocaine toxicity
E. Muscle tension

I'd say D is the answer, cocaine toxicity.

Well... I had to choose between carbon monoxide poisoning and cocaine toxicity. Both of them can have the same symptoms: headaches, confusion, movement disorders ("clumsy"), shortness of breath, tachycardia, reddish aspect of mucous membranes. But, if there was carbon monoxide poisoning, why didn't the wife report the same symptoms? She's been away on a trip and recently returned, but I don't know what "recently" means. Could be 2 days or 2 weeks... Could be enough time for her to develop the same symptoms ( if CO poisoning ). So, that's why I picked cocaine toxicity. I might be wrong... so I'm waiting for other oppinions...

this qs was posted earlier and i think answer was not clear than either. i believe it is COCAIN TOXICITY . reason is being monoxide poisoning will be acute and s&s developing acutely as well. his wife was gone away he might be having some fun with cocain. BUT problem with qs is there is no typical findings of cocain inhalation.mucus membrane turning red in cocain abuse , i never heard of it.

cocaine prevents DA reuptake so pt should be euphoric, hypervigilant n anxious but here d person is clumsy n confused...........
cud it be related to his acccident?
