A 65 year old diabetic male comes to the physician because of pain in his calf muscles. His pain increases with walking. He also has end stage renal disease, hyperlipidemia, and hypertension. His temperature is 98F, BP is 150/96mmHg, Pulse is 80/min, and RR is 16/min. Exam shows skin atrophy, shiny skin, and loss of hair on both legs below the knee. Which of the following would be the most appropriate next step in management?

A. Prescribe amitriptyline for his pain
B. Obtain Doppler ultrasound examination
C. Obtain resting and post-exercise systolic blood pressures in the ankle and arm
D. Segmental volume plethysmography
E. Obtain MRI of the spine

What about ASA and Exercise in this case? Would it help him like it did in "Intermittent Claudication" -first question I posted. ??

We all agree is ABI but why at rest AND Post Exercise ???
