A 68 year-old coal miner with decades of work-related exposure to dust is examinedfor pulmonary fibrosis. his FEV1 is 75% and his arterial oxygen saturation is 92%. his alveolar ventilationis 6000mL/min at a tidal volume of 600mL and a breathing rate of 12 breath/min. pathologic changes in lung comliance and residual volume are also documented in this patient. which of the following describes this patients lung copliance measured under static conditions a. change in ditending pressure(Palv-Ppl) devided by change in lung volume b.change in distending pressure(Palv-Ppl) minus the change in lung volume c. change in elastic recoil pressure (Palv-Ppl) d.change in lung volume devided by change in distending pressure(Palv-Ppl) e. lung volume devided by recoil pressure(Palv-Ppl) please explain

well i would go with C , since it is a restrictive disease, FEV1 is decreased, if they said pathologic changes in lung compliance are documented under static conditions the recoil force in fibrosis is increased, the lungs are stiffer so the lungs are less compliant

D Palv is ambient atmospheric pressure, or zero reference pressure, and Ppl is negative intrapleural pressure that becomes even more negative during inspiration.the lungs expand to a higher volume during inhalation as a result of an increase in the transpulmonary or distended pressure(Palv-Ppl).Static compliance is measured under conditions of no airflow.with each increase in distending pressure there is a corresponding flow incease in lung volume.

hey Dr C why are u thinking the diagnosis as restrictive disease? The total lung capacity is increased and the tidal volume is higher than normal (500ml/min). i think answer is D. If u disagree, pls explain the reason of your answer.

got tricked by the history of the patient, what he got checked for and the fev1, as the volumes are not decreased i shouldn't be thinking bout lung fibrosis, u r right.
