A 3 yr old boy is brought for follow up exam.. he just completed 10 day curse of amoxicillin that has not resolved his right ear pain. he appears irritable. his tempo is 102F. exam shows downward and lateral displacemtn of right auricle with tendernes to palpation of the posterior auricular area. his neck is supple. which of the folowing is most appropriate next step in Dx
A) bone scan
B) CT of head
C) tympanometry
D)LP
E) Tympanocentesis
is it B or E?
in kaplan i saw its writen CT as Dx.. but i read somwhere in answers as E... can somone plz explain?

no tympanic membrane data is included -- dont think goal of Q is to do tympanocentesis
ear pain after AB - not correct AB ?
but
tenderness behind ear -- possible mastoiditis
next step: CT

B

The clinical diagnosis is mastoiditis that is confirmed by CT (fluid-filled middle ear and demineralization of mastoid trabeculae)------->immediate treatment------>IV antibiotics.
