Physical exam has insufficient significance when used alone... It's currently a recommendation I, according to USPSTF... Mammography is indicated in patients over 40 years, without family history or known BRCA mutations... CT is too expensive...
In both cases I go with (B) USG

I thought mamography could be indicated for women beyond 35 years. So 35 is the cut off age according UW. Agree/disagree?

According to USPSTF 2007, mammogram is to be routinely ordered every 12-33 mo for women over 40 y/o... The USPSTF's judgment is based on the incidence of BC in a population of women before/beyond 40 y/o.

UW discusses this issue from a standpoint of the breast tissue density and a prediction of false negative/positive results in young females...

USG in both cases. As mammography after 40 years only and small breasts USG better to examine.

she is 29 not 40 and not 30, thats why I said PE
beside USG is not for BC screening.
No mammo dosnt have any value before 35 becuse the breast will be very dense and it wont have any sensitivity or specificity.

Ok Mamograms should be done every 1 to 2 years in women at average risk of breast cancer starting at age 40. (USPSTF & American Cancer Society)

But any women at any age with symptoms or signs of breast cancer should undergo work up for breast cancer...i don't have the info regarding the guidelines of screening in patients with strong family hystory of breast cancer if someone does pz post it.

We all agree that mamography for technical reason(breast density) should be indicated beyond 35 but is there any exception?

Mammo for Screening:
Baseline mammo @ 35-40
40-50 -every 1-2 years
50-75 yearly

If there is a FH of Breast cancer you can do the Mammo at 35,
The only contraindications are:
-Age <20 (breast too dense)
-Lactating (because we will only see milk)

Mammo CAN be done in pregnancy as well.

Its not the size of the breasts that matters; it's the age and FH.
There was a Q in UW where the woman was 35 with a strong FH and the next step was to do mammo.
But generally breast cancer screening starts around 35-40 years old. Before that risk of breast cancer is very low...

Here are the guidelines:
SBE monthly at 20years old 7-8 days after menses
CBE annually from 40

Also to add: mammo is usually NOT done past 75 years
