I believe it's Wegener G.:
- Necrotizing granulomatous ( giant cells , macrophages, epitheloid cells) arteritis

Agreed. Necrotizing arteritis and focal collections of epithelioid histiocytes are necrotic granulomas in small sized arteries. Histiocytes stand for macrophages. There is lung involvement without mentioning of renal involvement. Wegener primarily affects the lung.
