60 yr old after a lower abdominal surgery has suprapubic discomfort and feels the need to void but cant do so, u put a catheter in the bladder, now 6 hrs later pt still has not voided, whats the next step?

mechanical obstruction is the cause of this urinary retention. Do IVP

yes if pt has post op urinary retention , u pt a urinary catheter n leave it in n pt doesnt void, take it out n put another catheter

check that the bladder is distended, check that the catheter is patent, if so, then give iv fluids 300 cc of ns over 2-3 min

If some crucial info is not in the question stem, then its as good as anyone's speculation. What happened after initial catheterization is not given. So can we assume that the patient voided then? If so, was the catheter taken out after first time?.. If the catheter is still in there, then its obvious that a mechanical blockage is present in catheter. If the catheter was taken out, then a repeat and in situ catheter for 2 to 3 days may be kept as the pt improves (than doing repeated in and out cath)

If the patient has urge to void and has suprapubic discomfort from not being able to do so..his kidneys are working fine so far..I guess.. why do we need an IV saline challenge anyway? ... never mind..just for the arguments sake of the question...

Do KUB, try flushing the bladder w/ 3 way catheter & ns. Monitor renal fx.
