Hi,

I would really appreciate if someone can help me with following questions

1. What does a physical exam for residents consist of and are there any conditions that can prevent the program from hiring a person?

2. My understanding is that if a person's PPD ever comes +ve , he need NOT to undergo ppd test in future. Is that right? Can a + ppd but normal CXR in any way be problematic ?

If you meet criteria for treatment, they treat you. It's not a big deal.

well in my country we receive BCG vaccine at age of 1 year so almost everyone will have a +ve PPD but a negitive chest X-ray ! Never thought of that !

1. At my hospital, it's a drug screening, colour blindness, and PPD.

2. LOTS of residents have a positive PPD, they do a chest xray and move on. Not a big deal.

Nice to see you, How about visual acuity...do they check that too..like reading snellens chart

Regarding the original questiion, If you have a previous positive PPD evidenced by some health care professional, you might not have to take PPD ski test again. A negative CXR rules out an active disease suspicion so they will do a cxr again. You will have to take INH prophylaxis for 9 months.
The current concensus is BCG vaccinaion >5 yrs ago cannot give a significant Induration so everyone is recommended to take INH prophylaxis.
As BBB said, its no big deal. Many FMGs have positive PPD

I was vaccinated with BCG and I have a -ve PPD , so not necesarilly you will be +ve just because you were vaccinated.

listen, it's not only about whether the program will select you or not on the basis of that but it's also about your health....you should go to the primary care dr & discuss the situation with him.....it's very much treatable...they'll give a 6 mth single drug therapy & then u can show that to ur program + u'll be at [eace as well that there isn't anything REALLY going on inside that u should be worried about.

but with a negative CXR and a positive PPD...we have to take the INH prophylaxis right? what if we say no...??( am a FMG with a childhood BCG vaccination)

in the same situation as you... but i had to take the 9 month INH prophylaxis (as oulined in a post above)... it is now mandated by the Public Dept of Health

Not a big deal - just watch out for any side effects and pop your B vitamins

What if the resident have Hepatitis B or HIV? what happens to that resident?

Did you already had your physical and started the INH?

bbb...is the X ray mandatory aftr a positive PPD?i mean if one has a normal x ray reported within the past one or two years would that be enough? i guess wat im tryng to ask is that if a person doesnot want to get exposed with more radiation than he/she has already been exposed to then would a report of normal x ray save him the trouble to go through it again?

coca cola...were u previously treated for tuberculosis?what if one has already taken the 9 months course of drug>? suppose 2 to 3 ys back? will he have to take the prophylactic dose of INH for 9 months again?? and again as PG12alpha wrote..wat if v say no?!

Only if the negative xray was done within the past 12 months and you got the actual xray or a report by physician who deemed it negative.
Also depends if you have already completed INH prophylaxis.
My question is how many of you had positive ppd, negative cxr before but never took INH 9 mon for that?

My hospital is. It's a mandatory once a year thing. Even I have to do it and I don't see patients. All hospital employees have to have a annual health screening.

The way I understand this thread, many many interns have to take a INH proph for almost all of their intern year?? What about all the unnecessary SE? Coz most of us have been exposed to PTB, few have the active disease, and ALL have to take the drug?raised eyebrow

no I have not had the physical yet... this was for another job i held... had to do a PPD... knew i would come up positive because i always do... went to get an x-ray because the previous one I had was over five years old... thats when a rep at the dept of health told me the new regulations... got my chest x-ray - which was clean and started my therapy!

The pre-employment physical, etc, you can't do anything else but what the program/hospital wants you to do. Don't question why - just do it.

Does the program test you for HIV or Hepatitis ? What happens if one of those comes out to be +ve?
