This Certification of Identity is valid for this and any subsequent Step 3 applications submitted to the Federation within a period of five years from the date of the applicant's signature. If you do not sit for this adminstration of Step 3 or must retake Step 3, it is not necessary to submit another Certification of Identity as long as this form is on file with the Federation of State Medical Boards and has not expired. (Note: Forms prior to 2005 cannot be substituted for this form.)

I have submited once, but now, it appeared again to sign it. Where on the site can I check that the one that I have submited last year is stil available?

is the certification of identification different from the one we sent to ecfmg. do we have to send something to fsmb too?

If you apply for first time to step 3 yes, it is a form that must be autorizhed by a public notar. You can download it after you pay.

And to answer my own question, it does not need to be resubmited once you send it once, is valid for 5 years.

ok. so it can be notarised in the US itself or is it to be done in the home country?

mine arrived by post office. I don't remember if I got notifed by email.

I think any notary is ok, but you might want to ask fsmb. I got mine in my home country.

I suggest that you make 2 exemplars. it is always good to have copies of documents.

if you are in th US - you can go to any bank - they have always a notary - one of the bank people there will be certified as a notary public. just trying to help with my 2 cents advice

Agree with that. If you are outside of the US, then you go into your local US embassy and they will certify the form for you. Other notaries public cannot do that.
