Now there is a big surprise:

There is additional form added in J1 application ALSO MUST be filled by both applicant and program read this

A Training/Internship Placement Plan, Form DS-7002. All exchange visitor (J visa) trainee or intern visa applicants with DS-2019 forms dated on or after July 19, 2007 (based on Box 7 on form) must also present Training/Internship Placement Plan, Form DS-7002 when applying for your visa. If your Form DS-2019 is issued prior to July 19, 2007 a Form DS-7002 is not required

any one heard about this form before or contact ECFMG for it( ECFMG will issue both DS2019 and DS-7002) I am so confused

Do you think the PC or ECFMG aware with this form , or it is not applicable to us !!!

Looks like it's another form - I bet that ECFMG knows about this.

I went to the ECFMG web site and looked it up:

When applying for a J-1 visa at a U.S. Consulate, do I need to submit a Form DS-7002?

No. Form DS-7002 is a new requirement for J-1 Exchange Visitors in the “trainee” category only and does not apply to foreign national physicians seeking to enter the United States for the purpose of graduate medical education in the category of J-1 "alien physician."

I guess people who need j1 for residency do not need this form. Though I am not clear on what they mean exactly by "trainee" ?

ECFMG's Exchange Visitor Sponsorship Program has received numerous inquiries about a new form created by the U.S. Department of State, Form DS-7002. Form DS-7002 is a new document to be used for J-1 Exchange Visitors in the "trainee" category ONLY and will not be used for foreign national physicians seeking to enter the United States for graduate medical education in the ECFMG-sponsored category of J-1 "alien physician." Current regulations pertaining to J-1 physicians can be found in [22 CFR 62.27].

Perhaps a student. Either way, it's not resident physicians. You have enough paperwork to worry about without one more form.
