the intensity depends upon the pressure gradient between the LV pressure and systemic pressure when you do HANDGRIP....the arterial side resistance is increased. So the pressure gradient between the LV peak systolic pressure and systemic pressure is decreased and so it the SOUND while amylnitrate causes arterial pressure to fall down and there is higher gradient between peak systolic end LV pressure and systemic areterial pressure hence the sound increases hope it will help

Yes it help! Thank you WaqasQureshi! I knew that in left heart lesions(AS, AR, MS, MR) more blood in the heart ...more murmurs and this don't explaine decrease of murmur in aortic stenosis with handgrip, but your explanation make more sense!

Thanks for the qs and the explanation. I had this q too..and cudnt figure out why..now it make sense.Like valsalva/standing less blood -less murmur and squatting/leg raise more blood ,more murmur..but this wasnt clear.
