A 55 year old man is being treated fro PE. On the 5th day of hospitalization he is found to have a platelet count of 75,000/mm3. Heparin therapy is stopped immediately in this patient to prevent with of the following complications: A. Severe Bleeding B. Arterial Thrombus C. Liver Failure D. Vertebral collapse E. Heart Failure

B i think PLT still "high" to "bleed severe"

B Heparin-induced platelet agregation can be followed by formation of antiplatelet antibodies so you should discontinue heparin

BBBBBBBBBBBBBBBBBBBBBBBBBBBBBB dont be sad , there is another clinical conditions in which the labs contradict with clinical manifestations of the disease ...eg antiphospholipid antibody syndrome in which there is hypercoagulable state ( clinically ) but (labs) show prlonged PT and PTT>>>treatment is by heparin . in HIT treatment is by 1- stop heparin ...2 -use another anticoagulant agent that does not cause thrombocytopenia as hirudin ( contraindicated in renal impairment ) or argatroban ( contraindicated in hepatic impairment )

B is RIGHT! There was another question in which the patient developed HIT, but he presented with bleeding at the venipuncture sites... So HIT does cause bleeding as well as thrombosis (due to the antiplatelet Antibodies), but overall, thrombosis is more common than bleeding.
