I recently heard from a 2nd year med student that if you pass just your USMLE step 1 regardless of whether you are still in med school or not, that you are instantly considered a doctor. I do not think this is true. Can anyone clarify?

You are a doctor when you get a degree that calls you doctor.

The USMLE exams are just a licensing exam for the United States - they are used as a verification of medical training, in conjunction with medical school records and ECFMG (if necessary).

If you are from an IMG school that hs some dificyulties with step1, people might say that if yu passed step1 you are in your way to passed the other exams.
To consider yourself a doc, I do not think so, you might know a little thing here and there because of your preparation more than the doctors in your country, but for you to be coinsider a doctor like bb says you must MUST finish medical school.

Is like me, I'm not in residency yet but I feel I'm when the truth is I need to finish the exams. Now let me hit the books to become that a reality.

In the US, you are considered as a Doctor only if you have a license/or graduated from an accredited residency program.

Many people in the US have MD degree in their country but are not considered as MD unless they graduated from an accredited residency program.
