Do we need repeat the match process again next year and have nothing to do for 1 year after prelim?
this is the "safest" answer. i've met quite a few on the interview trail this year. but as i mentioned before, PGY 2 spots do spring up so you can go for those. monitor the forums, and keep your ears and eyes wide open. had i not matched in neuro, my game plan would be to rock prelims, ask the help of my PD and pray really really hard!
a year off will not be too bad, you can do research or just relax--imagine you'll have a sure post by next year! worse things have happened
