A 68-year-old woman with terminal metastatic breast cancer is living at home with her son. She has a living will requesting that she be allowed to "die in peace." She is unresponsive to voice and has not had any food or liquids for 3 days. Her son disagrees with her decision not to accept further therapy, including chemotherapy, antibiotics, hospitalization, and enteral or parenteral nutrition Which of the following is the most appropriate next step in management?

A) Abide by the decisions of the next of kin

B) Obtain a court order to allow additional therapy to be given

C) Start enteral feedings

D) Admit her to the hospital

E) No further intervention

---

do you admit her or let her die at home or what? I've said D - do you agree?

her decision = not to accept further therapy, including chemotherapy, antibiotics, hospitalization, and enteral or parenteral nutrition
So I go with E but I m not very confortable with my choice.

thank you. grrr....

I guess the tricky part here is that the lady is still at home, and probably the son is coming to the hospital to talk to you alone. I think this is not a house call and that the lady is not at the ER... grrr....

I would choose E
but the thing is if the patient is unconcius the nextkin or person in charge can do what they want right?

This is what I understand,

If the patient is unconscious, decision making is according to the following order:

1. Living will / advanced directive
2. Family member who is best representative of patient's wishes (may be next of kin)
3. Best interest standard
