age of kid, sex
onset of prob.
crying,listless, apathy,
feeding history( milk- solid food),
swollen tummy ,tummypain,constipation,diarhea,......
..any vomiting,any fever n runny nose , pulling of ear ,
n normal immunization n birthhistory..

yes BEANONCALLIDIOT should do most of the questioning and then write the diagnosis whatever comes to the mind according to mother's replies until n unless she is a GOMER= Get Out of My Emergency Room!!! grin
