hey guys here is a que for u smiling face

While working as the attending pediatrician in the neonatal ICU, you are called by nursing to examine a two day old male child who has recently begun vomiting. The child has failed to pass meconium since birth. Physical examination demonstrates significant abdominal distention. A rectal examination reveals no stool in vault. you strongly suspect Hirschsprung disease and inform the child's mother that an abdominal radiograph and barium enema are necessary. The mother forbids you from proceeding with any further testing or procedures because she does not believe " there is anything wrong" what is the most appropriate next step ?

A. Agree not to perform the tests

B. Obtain a court order to authorize testing

C. Proceed with tests

D. Contact the hospital's ethics committee for guidance

E. Wait until the mother provides consent

F. Call grandparents of child to obtain consent.

I will go with C as Hirsprung's has an 30% mortality rate if untreated and if there are delays in treatment there will be complications so I will go with C treat her waiting for courts will delay the situation or make the matters worse.

Guideline for this answer Kaplan Rule # 12 Ethics Chapter.

hey friends.

well try ! ok let me tell u the ans and its as per new_n_lost has said its "c".

questions like this where they give very close choices sometimes its really hard to decide which one to pick.

hirsprung's diz has the most common complication of perforation so that makes it a life threatening situation.

two close choices are to

1)obtain a court order and 2)contacting ethical commi.

but obtainin court order is time taking process. yes but contacting ethical commi. would be the correct apporach when ethical dilemma is present but above all these physician's first concern goes in the favour of child's safety interest.

and for that my dear friends doc must proceed with tests.

grandparents should be ligally involed with the child for making decission.

cyou friends
