According to UW Pulmonary Angio is the GOLD STANDARD, for diagnosing PE, but Helical CT is the "TEST OF CHOICE" because pulmonary angio is too invasive.

-would we have to choose from those two as the next best step in case of negative venous U/S?

I'd go for Helical CT. The algorithm goes like this

CXR and ABG --> EKG --> V/Q scan [ if option for giving heparin is given choose that] --> Helical CT --> Pul Angio.

Angio is obviously better but helical CT is cheaper and readily done.

IF THE BASLINE C XR IS ABNORMAL OR THE PTE. HAS RALES RONCUS OR CRACKLES ---SPIRAL CT SCAN
