I think - no. This is the first thing you do when come in - smile and shake hands. This is my plan: 1. Knock 2. Hi Mr.Smith, I am Dr.X, nice to meet you - shaking hands. 3. Next ask if he is comfortable and drape him. 4. Then I am not sure A. to ask what brought him today, sit, ask to take notes. OR B. Probably to sit first and then ask what happen, then ask to take notes. OR C. Sit, ask if it's OK to take notes, and then ask what is the problem.

For women: Wait for her to extend her hand before you do. (It's an etiquette manner) For men: Well, In real life I'd extend my hand to him before he did but in this test situation I'd rather let him do it first. (Let him know he has certain control and preference over your movements). I guess if he/she tries to shake hands, there's no need for washing hands since he is willing to do it.
