1 day old infant, possible sepsis, blood culture: gram + cocci in pairs, chains agglutinate with group B antiserum bacterial most likely colonization in mother's vagina can anyone explain it to me? thanks

What do you particularly want to know? Group B Streptococcus(gram positive coccus)is the most common cause of septicemia and meningitis in neonates.It colonizes vagina of 15-20% fo women and can infect the infant during labor or during delivary(increased risk when there is prolonged rupture of membranes).

Group B Strep - Strep Agalactiae (beta-hemolytic, catalase negative, bacitracin resistant) colonizes vagina. At week 36-37 gestation, pregnant mother is screened for this.

Group B septicemia in a neonate is a tragic occurrence that can be almost eliminated by proper prenatal care and Group B screening at the proper time. Our lab now performs PCR testing on Vaginal/Rectal swabs for Group B instead of plating, and positives are called and faxed to the ordering physician. We also perform the molecular testing STAT on L&D OB's who have not been screened. We had a case of Group B meningitis fairly recently - unfortunately the baby died. This was a needless tragedy.
