Hi,

My score is 221/92 in step 1

Exam date : Feb 11, 2008

Report date: march 5, 2008

I used all of the material which all examinees use for usmle like KAPLAN LECTURE NOTE, UW, Kaplan q bank and of course FIRST AID 2007.

But I would like share some of my experience with you. I think if someone give me these hints, I can get 99



these are some point:
-Just focus on one book on each subject and master it. of course when you start with Kaplan lcn , there is some difficulty because it is so brief and hard to understand so the best option is go to other book (like brs or high yield) just for understanding better as a references

-the time management is very important especially in 2 last block which usually encounter with very long question and you are tired because of duration of exam. I think the best option is practice. i took nbme 1 and nbme 2 and my score was 650 and 550 respectively, but in the real exam my score is decrease because after block 4 i got tiered and i don’t have time to read question and answer them(i chose around 20 question by chance).

-i think reading fa alone is note helpful .i saw a lot of people complain of bad format of fa .and i think it is true if you want to start with fa .

My opinion is first study for all concept from Kaplan and then go for fa and i believe you understand it better .
- the book that i strongly recommend is medessential of kaplan which you can buy from bookstore .although i think the physiology part is a little bit tense and more than enough but some part of this book is very good if you study it beside kaplan lecture note.

-for q bank i think uw and kaplan q bank is the best .nbme is also helpful.

for uw and kapan you just write a note from each test and review them for at least 2 times .start with kaplan q bank which is easier and give you some confident and then go for uw .make a note from uw and review your note in last two weeks before your exam.

the new part of kplan q bank (add some point in the end of each question started in 2008) can help you to review the important part of kaplan lecure note.

answer all question randomly in kaplan and uw question in time base format and then review the question in system or subject base.
(it help you to organize your brain to learning faster and better).

-don’t study one day before the exam .just relax and let your brain to organize the information.

-in first block, i find out the block is very hard may be because i am nervous or low confident or may be because my engine (brain) hasn't been warmed up yet.

So for first block answer the easy one (may be 30 from 50 ) and then come back and answer the hard one. you feel more comfortable .

-after the exam usually you just remember the question which answer wrong on the exam because the easy question or the question which you know are very easy to forget. in other hand, hart to remember after exam the easy question .

so don’t crazy and feel sad you did better han you feel after exam.

it dose matter how many book you study for each subject .but it is important how many times read one book and master to this book


some study book beside kaplan lecture note:

-read high yield neuroanatomy and know all the ct scan mri especially the first chapter. I have a lot of question from ct scan ,mri and cross section of spinal cord

-listen to Goljan lecture and also see his slide(it help you not only in step one but also in step 2).

-if you have step 2 lecture note in psychiatry study the epidemiology part beside the kaplan behavior science lecture note

-dot forget to read the 3 end chapter of kaplan

microbiology.

(Table and disease.)







-these are some subjects and questions that exactly appeared in my exam.



- Apoptosis and cytochrome c.

- free radical CCL3.

- know case control and cohort study (i ha two easy question).

- know sensitivity and specificity (decrease upper limit of normal of test what happens to false positive and false negative).

- Many question from doctor -patient relationship.

- Question from amino acid metabolism maple syrup, phenylketonuria.

- One question from stop codon.

- t-RNA structure and anti-codon

-2 question about enzyme kinetics (very easy just know the diagram)

-patient with pleural effusion on anticancer drug effect which part of cell cycle(I am not sure may be doxorubicin which cause heart failure and pleural effusion and effect most g1)

-picture of melanoma

-effect of glycogen on camp and fructose 1,6 bi-phosphate.

-know all hormone second messenger (5 or 6 question)

- know effect of adrenergic drug and receptor(dopamine, dobutamine ,isoproterenol)

-know lipoprotein LDL receptor defect in hypercholesterolemia

-Pb inhibits ALA dehydrase

-pation ??? have positive southern blot and northern blot but negative western blot why? decrease in translation(low protein)

-one autosomal dominant diagram (very easy)


-Duchene dystrophy and Gowner maneuver << Did you mean Gower’s sign ??

-Huntington disease (describe patient with progressive dementia and CT scan on brain and choose caudate atrophy in ct scan)

-B12 deficiency (describe patient and see blood smear big RBCs compare with WBCs )

-methanol intoxication rx: ethanol infusion

-vitamin e and antioxidant effect

-one question from sporothrix fungus (very easy)

-question about tuberous sclerosis and rhabdomyoma in heart in kids

-ANS dorsal root ganglia crania nerve originate from neural crest

-describe patient with pain LUQ and ichter ?? obstruction in common bile duct

-antibiotic resistance aminoglycosides and vancomycin mechanism

-Bordetella Pertussis and stimulation adenylate cyclase

-3 case of TB just know TH-1& TH -2 and all hypersensivity mechanism and also all cytokines IL -1, IL-2 and TNF -ALFA(i have 3 question from just cytokine)

-Hepatitis B Antigen + and hbe antigen + and ask what is patient have.

-one question about aids and ccr5

-polio symptom describe and then ask about viruses non envelope rna + virus

-one question about mononucleosis

-one question about rubella

-on question about transformation dna transfer to bacteria (note transformation in bacteria just describe dna go to bacteria

-spherocytosis with blood smear (very easy just see no central pallor in RBCs)

-basal ganglia question(just know the pathway )

-12 nerve problem(tongue deviate to the same side)

-on epidural hematoma question just normal and usual manifestation describe

-upper motor neuron problem (increase spasity)

-mastectomy and problem with long thoracic nerve

-show bilateral hemorrhage in axial occipital lobe and ask about eye filed defect

-cocaine intoxication

-bulimia nervosa case

-patient describe as manic what may be happen next major depressive episode(bipolar 1)

-agoraphobia problem to go to open place not close place (used this to confuse you) and problem to go out of house.

Personality disorder borderline (divided people to good and bad) in the stem of question

-2 Parkinson question and drug

- Hyponatremia and small cell carcinoma in lab profile

-anti seizure drug two question phenytoin

-Flumazenil effect competitive antagonist of GABA a receptor

-kidney scar :chronic pyelonephritis not chronic nephritis (confused you)

3 question from vitamin d and calcium(easy)

-acute tubular necrosis and granular cast

-one question about ovary cancer and one from breast cancer easy you can find out with age

FSH decrease in menopause than LH

diagram of progesterone in secretory phase(increase)

one case from endometriosis relation of pain with menses

-DIC case easy all PT PTT increase

-vWF disease

-cell lining in duct of breast: I think simple cuboid

2 question about ace (one Conn’s syndrome)

-pneumothorax question just know the criteria in physical exam

aortic aneurysm figure and describe ask about risk factor atherosclerosis

-know diabetic very well(i have many question)

-brain ct scan and label many part and ask about which receive sensory input chose thalamus

-one question about hydrocephalus in kids that obstruction in cerebral aqueduct which ventricle expand (lateral and third)

-very hard question about spinal cord tumor don’t forget ependymoma and histology .

-ct scan of chest show emboli in pulmonary artery and ask about history of immobility

-on question about anatomy of uterus and ligament

-show action potential diagram and which part are most conductance to Na and K phase 4 (I am not sure)

-ask about a criteria for malignant (chose metastase the othe like capsule and other option is not correct) ???

-one question about AML L3 (promyelocytic) and Auer rod

-know anatomy of hand and foot very well(3 question)

.2 question about secretion action in GI

MCAD deficiency(fatty acid metabolism) one question
