Lets say one were to obtain a surgical prelim position through the scramble and work on a J-1 visa, would it be difficult to apply for a categorical IM position the following year (on J-1)? Or, would it be an advantage since you will now have one year of USCE?

It's going to be difficult in now you have to interview for IM program during a very busy surgery schedule. Yes, you will have some USCE, but you still have to take vacation time for interviews, travel, pretend that you are interested in every program. Difficult - yes. Impossible? No.
