give diuretics to reduce the pul edema, other druge for pul edema are NOMAD Nitro 02 morphine aspirin diuretic

as for the endocarditis question and allergy to pen i would give clinda and azithro also poss is 1st gen ceph and clarithromycin

empiracal treatment for native valve --naficilin or oxacillin50 mg iv+gentamicin1.0 mg/kg iv every 8 hrs empiracal treatment for prosthetic valve is -vancomycin 15mg/kg+gentamicin1.0 mg/kg+rifampin 600mg prophalaxis for dental or minor GIor GU without prior history of endocarditis ampicilin 50 mg /kg.if its penicillin allergic then clindamicin20mg/kg or azithromycin 15mg /kg or clarthomycin 15mg /kgor cephalexin or cefadroxil prophalaxis for major GI or GU without prior history of prior history of endocardits---ampicillin 50 mg /kg if pt is pencillin allergic then use vancomycin 20 mg /kg IV

numbers are too invasive for step 2 but ur right clinda azithro clarithro cephalexin choice d and that is my final answer lol
