*Swollen ankles, Height: 5'9'' Weight: 244 pounds Age: 43
Bp=145/84 RR=20/min.

*hands & ankles swell also
*face swollen, frequently urinates--no blood
*1 to 2 six packs a day
*occupation: bartender
*associated symptoms: loses breath
*Noticed it a couple of months ago
*weight gain (50 Ilbs. in last few months)
*couple of years ago, used illicit drugs like cocaine
*no prescription meds.
*shortness of breath---after climbing stairs
*had gonorrhea & syphillis
*few years ago, doctor diagnosed him with hepatitis


HEENT nml, dullness to percussion in both lungs
*no crackles, heart nml
*pitting edema in both extremities
*left lower leg bigger than right
*pain on compression of calf & dorsiflexion of foot
*rectal exam normal, CXR--pleural effusion
*Chronic venostasis, RBC's in Urine, Positive Lupus Anticoagulant Test

What is the diagnosis?

and also the patient has albuminuria

Glomerulonephritis (membranous or membranoproliferative)

hepatitis with glumerulonephritis
