In which of following joint disease would you expect to find in synovial fluid a normal to slightly elevated neurtophil count and mucin clot study?

A. Osteoarthritis

B. RA

C. reiter's syndrome

D. ankylosing spondylitis

E. lyme's disaese

I thought long and hard on this one because the the question asks you the Joint disease with NORMAL to ELEVATED PMNs AND MUCIN CLOT

RA was my initial answer but it struck me that there is a poor mucin clot formation which then totally confused me either i'm missing something or i got it wrong.

A. Osteoarthritis
Normal to slightly elevated WBC count(200-2000 WBC/mm3)and a good mucin clot indicates a non-inflammatory conditions like osteoarthritis,trauma,degenerative joint disease,etc.
Highly elevated WBC count and poor mucin clot indicates inflammatory conditions(2000-50,000/mm3) which include choices B,C,D,and E OR septic arthritis(usually >80,000/mm3).

Yup I agree it should be OA cause its the one with a low or slightly elevated PMN count.

yes right.welldone!

High neurtophils but normal mucin clot --->OA and neuropathic arthropathy are two noninflamatory dis with this picture.inflammatory joint dis would have low mucin clot .

normal neurtophils-- <200 cells in SF

mucin clot-->is measure of hyaluronic acid in SF.
