I need your advice. A program has offered to rank me very high for a preliminary Internal Medicine position and since I like the program I'll be ranking it high.

Now I heard from a friend that if I want to apply for an Internal Medicine Categorical PGY1 position next year I'll have trouble since most programs don't accept applications from interns - because of some funding issues. Is this true?

I'll be on a J visa. I just have three days to make any changes to my ROL so would appreciate your advice - and anybody else who knows about this. Thanks a lot.

I am no BBB, but i heard from different sides and people, if you want to do IM (all 3 years), don't go for preim IM, because it will not go toward your PGY-1, and if you will go to IM, you would have to start your PGY-1 all over again and in this case there could be those kind of problems that you mentioned.
prelims are for those who go into other specialties and need 1 year of IM as a prerequisite, and if you plane for such program (it will be PGY-2 then after you are done with prelim year) there is no issues of that sort
it all depends on your future plans

Thank you so much babydoc. I was thinking of doing a prelim year followed by Int Med preferably at a University program. But then I heard about the whole funding issue that programs have. Now I'm confused if I should rank this program high enough.

don't be confused, just shift your priorities.... what is the reason that you want to go into university program? and to lose a year for it?

well, there are many issues involved along the way....
starting with losing 1 year being on visa.... is it something (i mean fellowship) that you absolutely must have? or is it something that looks pretty and prestige? or is it something that you have to prove to someone? yourself?
when you make a carrier choice, you have to be extremely honest about few things: why is it that you want it? and what are you willing to give up for it?
i have heard about quite a few people who wanted to go for fellowship, but change their mind along the way, because, as they put it, "was not worth it"....
if you have a good shot in a good community program (or university affiliated), where you feel comfortable and have environment which would promote your excellence, where you could grow and master your skills without looking over your shoulder, where you could invest in yourself, put 100% effort AND HAVE GREAT RESULTS, and don't have to play politics..... doesn't it worth it?
and what would you have i any university program that you can not have anywhere else except name and illusory chance of fellowship that you might not want at the end?
i know, i might sound a bit.... unconventional, and many people would say: why I SHOULD NOT want to go university program? and they may be right....
I just put my 2 cents (in form of my thoughts)smiling face, but ultimately it is you who have (or don't) all the reasons and motives....
go through them, and make a choice.... and stick to it

is going into surgical prelim a bad idea as well if one has no choice (ie has to scramble) would this also cause funding problems and make it more difficult to get into a cat PGY 1internal med program. My main priority is to get into an internal med residency program. My chances of matching this year are extremely remote. I was wondering whether i should scramble for a surgical prelim position or wait for one year for next years match. The reason i believe i didnt get many interviews cos i got my CS results and ECMFG certification pretty late (mid feb). any advice?

doc 1981-dont take prelim coz next yr its gonna b next to impossible for u to get categorical in im........u might hav to change speciality or if ur lucky , u might get an open py2 spot.........but these r not easy.......trust me , i know from a very close frnd........plus if ur on j-1 visa.....its gonna b for only a yr and god forbid , if things dont work out.........after a yr , where will u get an extension from?

go for categorical right now and save urself a crisis later.......gl

Thanks so much for the advice and suggestions babydoc, silenced and busybee. Appreciate it. I don't think I wanna get entangled in a nightmare scenario where after one year of prelim I have nowhere to go!
