Should the LOR be written essentially on an institution letter head.Because i've got some with the letter writer's name only printed on the letter. Any one applied in this way and the letters have been considered? Please do let me know. Anxiously awaiting your reply!

That's not a must it is a plus... at least get a letterhead from the doctor's office if you can.

Yes, every letter has to have an LOR submission form, (one for each letter) with all your info and the LOR writer's info and your signature; put it in front of the letter and join them with a paperclip NEVER WITH STAPLES!..... send them together in one envelope, if you are sending with all other documments you can do so and just join them all with a paper clip and also add the documment submission form on the front. If you are only sending one letter no need for this form only the LOR submission form. If you are waiving your right then the doctor him or herself has to send the letter and you should not see it so give the form to him or the secretary and ask them to send it with it on the front. Hope this helps, good luck!

if you did not waive your right to see the letter, you can mail it to ERAS yourself, like GloMD said, each LOR has to have a LOR submission form on top of each LOR..so in your case, 4LORs with 4LOR submission forms, that you can send all together in one envelope..

I'm sorry to bring this up again. I have got two home LOR'S . The only problem is the Professor's name and credentials with the name of college is printed on the top without the hospital 's name and logo at the top.Is this format ok? Please please let me know.Anyone been called for interviews with this format of LOR. Thank you.
