NEJM has an awesome impact factor (above 40)...only NATURE comes close to it...but I wud say getting publication in NATURE is more prestigious than getting one in NEJM. your options are not right. Options shud be - NATURE, NEJM, JAMA, LANCET, Circulation, Annals of internal Medicine, archives of internal Medicine. The first two are the best... and that too by some margin.

the "Best" probably depends if you are are researcher or a practitioner For me it's simpler: Clinical Medicine: NEJM, The Lancet, and Annals of Internal Medicine Basic Research or Life Sciences: Nature and Science... Major Clinical Studies are generally published in the NEJM, and The Lancet... JAMA (has good guidelines too) .....New Discoveries generally in Science or Nature ......During Internal Medicine training: My guess is I'll read mostly nejm and uptodate
